Our office accepts most PPO insurance plans. We do not accept Medicare or Medicaid plans.

We accept cash, debit card, Visa/Mastercard, , and personal checks.

Financial policy

If you have an insurance plan: Co-payments, Deductibles and or Co-insurance will be collected at the time of the visit.

Patients who present for Well-Woman exam who has a separately identifiable medical problem, may also receive a charge for that additional service and/or procedure. These additional charges are not covered as preventative thus are subject to copays or deductibles. It is best to resolve any medical issues separately from the annual check-up.

Medical Record Request. Please be aware that there may be a minimum charge of $25 for the first 20 pages of medical records, and .50¢ for each page thereafter. The final charge depends on the number of medical records pages.

Please allow 7 -10 business days to process your medical record request.

No Show Policy

We reserve the right to charge a $25.00 fee for NO SHOW appointments. To avoid this fee, please call our office to reschedule or cancel your appointment at least 24 hours before your scheduled appointment. This fee is NOT billable to your insurance company and will be your responsibility.

Out-Of-Network

We can see you as a self-pay patient if we are not on your insurance plan. Payment in full for all services is expected at the time of service. Since we are not part of your insurance plan, we are not obligated to submit any insurance for you nor conformed to any of its fee schedule or requirements.

Self-Pay

Payment in full for all services is expected at the time of service for patients without insurance coverage. Additional charges could be billed to the patient if the laboratory has to run additional tests or screenings due to abnormal or reactive results. A discounted fee schedule may be given to self-pay patients
